We also accept most major HMO plans, Please note that most HMO plans require you to have a referral from your Primary care before you can be seen.

We accept cash, personal checks, and most major credit cards (Master Card, Visa and Discover) as well as “Flex and HSA” cards. There is $3.95 convenience fee to accept debit or credit cards up to $200.00. All charges over $200 will incur a 3.95% convenience fee to accept the payment by debit or credit card.


We have options; If you do not have insurance our office visits begin at $70.00.

Please note: At the time of your visit you will be responsible to pay your specialist co-pay, coinsurance, past due balances and/or any unmet deductible amounts at the time of service. We are also happy to file your claim for you, but the balance is your responsibility if your insurance company or plan does not pay after 45 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.

Please make sure to verify that your insurance plan is in-network. We encourage that you also become familiar with your insurance plan benefits as benefits may change. It is your responsibility to know your benefit coverage.

For all visits we bill your insurance provider directly. If your insurance has changed please notify or provide us with your current information prior to your scheduled appointment. Please make sure to bring your insurance card with you to your appointment; we will need to take a copy of the new information printed on your insurance card.

Please be aware that you are required to disclose all insurance plans regardless of the coverage. Failure to do so will result in denial of your claim and you will be responsible for the payment.
All claims have a time cap on them and must be submitted with all your insurance plans or they will get denied.
